The 2024 Telehealth Trailblazer award was presented to Caitlin Moore, MS, APRN, FNP-C, during the 12th Annual Telehealth Summit of South Carolina held last month in Hilton Head. The Telehealth Trailblazer Award recognizes an individual or organization that is leading the way utilizing telehealth to improve access to care.
Moore is Associate Director of Clinical Operations at Clemson Rural Health, which serves patients in Abbeville, Orangeburg, Clemson and Walhalla. Clemson Rural Health also provides healthcare from five custom built solar powered mobile units that bring a physical exam room to the most rural areas of the state, breaking down the barriers to care.
Moore, in partnership with Palmetto Care Connections, has incorporated telehealth platforms into her work at Clemson Rural Health as well as provided Remote Patient Monitoring to her patients.
“The most important thing to know about Caitlin is her passion. She is a passionate person and cares deeply about health, particularly rural health,” said Leslie Hossfeld, PhD, Dean of the College of Behavioral, Social, and Health Sciences at Clemson University.
Palmetto Care Connections is the host of the Annual Telehealth Summit of South Carolina. The Telehealth Summit brings together leaders and professionals interested in utilizing telehealth to increase access to health care.
